A person once asked me a very simple question: “If warm water is giving me relief, can I just keep taking sitz baths and wait for my fistula to heal?”
This question matters.
A sitz bath can be very useful when you have pain, irritation or a wound around the anal area. It can also be part of care after fistula surgery. But there is one thing I want to make clear right at the start:
A sitz bath can help with comfort and hygiene. It does not usually cure a perianal fistula.
An anal or perianal fistula normally needs medical assessment, and many cases need surgery because fistulas rarely heal by themselves.
So use warm water as supportive care, not as a replacement for your colorectal surgeon’s treatment.
What Is a Perianal Fistula?
A perianal fistula is a small abnormal tunnel between the anal canal and the skin near the anus.
It commonly starts after an infection.
An infected gland near the anus can develop into an abscess. When the pus drains, a small tunnel may remain between the inside and the skin. That tunnel is the fistula.
People may notice pain, irritation, swelling or discharge. Pain can sometimes get worse while sitting or during a bowel movement.
If this sounds like your problem, dont try to diagnose it yourself.
A doctor may need to examine the area. Some people also need an ultrasound, CT scan or MRI to understand where the fistula goes.
Can a Sitz Bath Help a Perianal Fistula?
Yes, but we need to understand what “help” means.
Warm sitz baths are used to make the area more comfortable and keep it clean. Current hospital guidance for people with anal fistulas recommends warm sitz baths for around 10 to 15 minutes, along with keeping the area clean and dry.
Warm baths may also ease anal pain.
This can make a difference when sitting, walking or going to the toilet feels uncomfortable.
But warm water is not closing the fistula tunnel.
That difference is very important.

Why a Sitz Bath Does Not Replace Medical Treatment
I feel health products should be explained for what they actually do.
If I sell you a sitz bath and tell you it will make a fistula disappear, I am giving you the wrong idea.
Anal fistulas generally dont heal on their own. Surgery is often needed. The treatment depends on where the fistula travels and how much of the anal sphincter muscle is involved.
One treatment is fistulotomy, where the fistula is opened so it can heal.
Another approach uses a seton, which is surgical thread placed through the fistula to help it remain open and drain. Other options can include LIFT and advancement-flap procedures.
The doctor chooses treatment based on your fistula.
So please dont delay medical care just because warm water is making the pain feel better.
Sitz Bath After Fistula Surgery
This is one area where sitz bathing can become very useful.
After anal fistula treatment, colorectal specialists commonly include sitz baths as part of recovery at home. They can help with comfort and local hygiene.
A newer hospital patient guide, reviewed in July 2026, also advises warm sitz baths for 10–15 minutes as part of caring for an anal fistula.
Your own surgeon’s instructions come first.
One surgeon may ask you to bathe after bowel movements. Another may give a different routine depending on the procedure and wound.
Follow that advice rather than copying somebody else’s recovery plan.

How to Use Warm Water Safely
If your surgeon has advised sitz bathing, follow the timing and frequency they give you.
The water should be pleasantly warm, not very hot. Colorectal guidance describes warm water at around normal bath temperature and warns that excessively hot water can injure the skin.
Keep your sitz bath clean before and after use.
After bathing, gently dry the area rather than rubbing it hard. Daily bathing or showering and gentle drying are also part of post-treatment guidance. Perfumed soaps and gels may sting the area.
I would not start adding random antiseptics, oils or other products to the water unless your doctor has asked you to.
Simple care is often better.
Keep Bowel Movements Easier Too
People sometimes focus so much on the bath that they forget what happens when they go to the toilet.
Hard stools and straining can make an already uncomfortable area feel worse.
Hospital guidance for fistula care recommends a high-fibre diet and plenty of fluids.
Your doctor may give you more specific advice after surgery.
If you were prescribed medicine, antibiotics, pain relief or another treatment, follow the instructions you were given. A sitz bath should sit beside your medical plan, not replace it.
When Should You Contact a Doctor?
Dont keep waiting at home if things are getting worse.
Seek medical advice if pain is increasing, you develop fever or chills, there is heavy bleeding, or you notice increasing redness, swelling, warmth or other signs of infection.
Fistulas can also come back even after treatment, so returning symptoms should be checked.
If you have ongoing pain, discharge or swelling and have never been diagnosed, get examined rather than assuming it is piles or a fissure.

Warm-water sitz bathing can help with comfort and hygiene, especially during recovery. But the fistula itself often needs proper medical treatment.
