Heal Better. Feel Better
A question we get from people using a sitz bath is:
“How much Epsom salt should I put in the water?”
Many people think Epsom salt is a required part of every sitz bath. It isn’t.
I am Abhinav Jain, founder of The Heal Company, along with Deepika Jain. At The Heal Company in Delhi, my preference is to keep sitz bathing simple. For most people, plain comfortably warm water is enough unless a doctor has specifically asked them to add Epsom salt or another ingredient.
Medical guidance supports this approach. Warm water alone can provide the main benefits of a sitz bath, and adding salts, oils or other products may irritate sensitive skin in some people.
So before talking about an Epsom salt ratio, there is one thing you should know:
There is no single medically accepted Epsom-salt-to-water ratio that is right for every sitz bath and every health condition.
Do You Actually Need Epsom Salt in a Sitz Bath?
No.
This surprises many people.
A sitz bath is mainly about soaking the perineal or anal area in warm water. The warmth may help relax the muscles and can provide relief from pain, itching and irritation associated with some conditions.
Epsom salt is magnesium sulfate. You may have seen it sold for soaking in baths.
But adding more salt doesn’t automatically make a sitz bath more effective.
Some medical guidance specifically recommends warm water alone because salts and other substances may cause irritation.
So if your doctor hasn’t asked you to use Epsom salt, plain warm water is the simpler option.
So What Is the Correct Epsom Salt Ratio?
This is where internet advice can get confusing.
You may find websites giving one tablespoon, two tablespoons, half a cup or even more. The problem is that the amount of water being used is often different.
Medical instructions also vary based on the reason for the sitz bath.
For example, one hospital’s post-surgery instructions allow 1 tablespoon of plain Epsom salt in bath water for a specific group of patients. Another set of procedure-specific instructions uses a different amount.
That tells us something useful:
Don’t use a random universal ratio.
If your healthcare professional recommends an Epsom salt sitz bath, ask them for the amount based on:
- your medical condition
- the size of your sitz bath
- how much water you use
- whether you have stitches or a wound
- whether you’ve recently had surgery
- how often they want you to soak
That is safer than copying a ratio made for somebody else’s condition.

How to Add Epsom Salt When Your Doctor Recommends It
When a healthcare professional has specifically told you to use Epsom salt, follow their amount and frequency.
Start with a clean sitz bath.
Add comfortably warm water. The water should never feel painfully hot.
Measure the exact amount of Epsom salt your healthcare professional recommended. Add it to the water and let it dissolve before you sit down.
Then use the sitz bath for the time your healthcare professional has advised.
Afterwards, gently pat the area dry rather than rubbing it.
General sitz-bath guidance commonly describes soaking for around 10–20 minutes, but your own doctor’s instructions should come first.
Why More Epsom Salt Isn’t Better
This is probably the biggest mistake I see in home-care thinking.
Someone thinks:
“If one spoon is good, four spoons must work better.”
That logic doesn’t work here.
The skin around the anus and genital area can be sensitive. Salt or other additives may cause irritation for some people.
You are trying to make the area comfortable, not create a stronger mixture.
If the water starts causing burning, increased irritation, redness or worsening discomfort, stop using the mixture and speak with a healthcare professional.
Epsom Salt Sitz Bath for Hemorrhoids
People often search for Epsom salt when they have hemorrhoids.
Warm sitz baths are commonly suggested as one way of managing hemorrhoid discomfort. Warm water can help with comfort and relaxation around the area.
But Epsom salt itself isn’t required to get the basic benefit of a sitz bath.
If you have hemorrhoids and your doctor hasn’t told you to add anything, you can ask whether plain warm water is suitable for you.
The same thinking applies to The Heal Company – Sitz Bath Tub Manual. The product gives you a convenient way of taking a sitz bath. It doesn’t mean that salt must be added to the water.
What About Anal Fissures?
Warm-water sitz baths are also used for anal fissure symptoms.
Medical guidance describes taking a shallow warm-water sitz bath for around 10–15 minutes, two or three times a day, as one way to relieve symptoms and relax the muscles.
Again, notice the main ingredient:
Warm water.
If you have a fissure and want to add Epsom salt, ask your doctor first. A fissure is already a painful break in the skin, so I would not experiment with different mixtures on your own.
After Childbirth or Surgery? Ask First
This is one situation where I would be extra careful.
After childbirth, stitches or surgery, don’t assume that Epsom salt is automatically suitable.
Postpartum medical guidance recommends warm sitz baths for some kinds of perineal discomfort while also advising people not to add products such as bubble bath because of infection concerns.
Other procedure-specific medical instructions sometimes do prescribe Epsom salt.
These recommendations may sound different because the patients and procedures are different.
Your surgeon, gynecologist or other healthcare professional knows what happened to your body and can tell you what belongs in your bath.

Whether someone is using our product in Delhi, Mumbai, Jaipur, Ahmedabad, Bengaluru, Chennai or another part of India, my advice about additives stays the same:
Don’t add Epsom salt just because the internet says every sitz bath needs it.
It doesn’t.
Will Epsom Salt Make You Heal Faster?
I would not make that promise.
The main medically described action of a sitz bath comes from the warm-water soak. It may relax muscles, improve local blood flow and provide relief from discomfort.
That doesn’t mean adding Epsom salt will automatically make hemorrhoids, fissures, postpartum wounds or surgical wounds heal faster.
At The Heal Company, I would rather give a customer a realistic answer than turn Epsom salt into some magic ingredient.

Plain Warm Water Is Often Enough
When Deepika Jain and I started The Heal Company, our idea was to make home-care devices simple to use.
A sitz bath doesn’t need ten ingredients.
For many people, it can simply be:
Clean tub + comfortably warm water + correct sitting time + proper cleaning afterwards.
If your doctor says to add Epsom salt, use the exact amount they recommend.
If your doctor hasn’t asked you to add it, don’t feel that your sitz bath is incomplete without salt.
Sometimes simple really is better.

Medical note: There isn’t one universal Epsom salt ratio for every sitz bath. Plain warm water is commonly recommended, and salt or other substances should be added when your healthcare professional recommends them for your particular condition.
