Potassium Permanganate Sitz Bath: Medical Use Guide

By Abhinav Jain, Founder – The Heal Company

Heal Better. Feel Better

A person once asked me, “Abhinav, my doctor mentioned potassium permanganate. Can I simply put some into my sitz bath at home?”

This is one question where my answer is very different from normal warm-water sitz bath advice.

Please dont experiment with potassium permanganate yourself.

I am Abhinav Jain, founder of The Heal Company along with Deepika Jain. Potassium permanganate is a real medical chemical with antiseptic and drying properties when correctly diluted. But concentrated or incorrectly prepared potassium permanganate can irritate or burn skin, and swallowing it can be extremely dangerous.

So this article is about understanding its medical use — not a recipe for mixing potassium permanganate at home.

What Is Potassium Permanganate?

Potassium permanganate is a chemical used in some medical skin-care and wound-care settings.

When correctly prepared by healthcare professionals, diluted potassium permanganate acts as a mild antiseptic and astringent, meaning it can also help dry wet or weeping skin conditions. Medical guidance describes uses including certain weeping or infected eczema, wounds and blistering skin conditions.

It is normally recognised by its purple colour.

But dont think:

“If a little is medically useful, more will work better.”

That can be dangerous.

Can Potassium Permanganate Be Used in a Sitz Bath?

There is historical and medical guidance describing potassium permanganate with sitz baths in certain situations.

For example, Rajasthan’s Standard Treatment Guidelines list a lukewarm sitz bath with potassium permanganate as one non-drug measure for fissure-in-ano, alongside local hygiene and a high-fibre diet.

But this should not be read as permission to buy concentrated potassium permanganate and start mixing it yourself.

More recent medicine-safety guidance says potassium permanganate’s place in treatment is not described uniformly in medical literature. Healthcare systems have also introduced stronger safety controls around its prescribing, preparation and storage.

For me, that changes the answer completely.

Use potassium permanganate only when a doctor or other qualified healthcare professional has told you to use it and has given you clear instructions.

Why Correct Preparation Matters So Much

This isnt like deciding whether to put normal salt in warm water.

Potassium permanganate is a strong oxidising chemical.

If the solution is too concentrated, medical guidance warns it can cause skin irritation, redness, pain and chemical burns. It can also irritate sensitive tissue and stain your skin, clothes and bathroom surfaces.

Because this chemical can cause harm when prepared incorrectly, I am deliberately not giving a DIY measurement or mixing formula here.

If it has been prescribed for you, use only the preparation and instructions supplied by your healthcare professional or pharmacist.

Never Swallow Potassium Permanganate

This part needs to be very clear.

Potassium permanganate is for external use only. It must never be swallowed.

A national patient-safety alert was issued after cases where people accidentally swallowed potassium permanganate preparations. Concentrated products may look like ordinary tablets or liquids, which creates a serious risk of confusion. Ingestion can cause severe harm and can be fatal.

Keep it away from children and never store it where someone could mistake it for medicine, food or drink.

If potassium permanganate is accidentally swallowed, seek urgent medical attention immediately.

Potassium Permanganate Sitz Bath for Anal Fissure

This is probably one reason people in India come across the term.

Indian standard treatment guidance has included lukewarm sitz bathing with potassium permanganate among non-drug measures for fissure-in-ano. The same guidance also discusses local hygiene and preventing constipation through a high-fibre diet.

Still, an anal fissure isnt something I would suggest treating by experimenting with chemicals.

Warm-water sitz baths are commonly used as supportive care for fissure discomfort. Your doctor can decide whether anything needs adding to that water.

If pain or bleeding continues, get checked rather than simply making your bath stronger.

Is Potassium Permanganate Good for Piles?

People also search for potassium permanganate sitz baths for piles or hemorrhoids.

Here I would separate two things.

A warm-water sitz bath is commonly used to help with hemorrhoid discomfort.

Potassium permanganate is a medical chemical with particular topical uses.

Those facts do not mean everybody with piles needs potassium permanganate.

In my opinion, dont add it to a hemorrhoid sitz bath unless your healthcare professional specifically recommends it.

Piles can also involve bleeding, and unexplained or repeated rectal bleeding deserves medical assessment.

What About Postpartum Sitz Baths?

After childbirth, the perineal area may be tender. There may also be stitches or healing tissue.

This is not the place for DIY chemical experiments.

If your obstetrician, gynaecologist or maternity team recommends a plain warm-water sitz bath, follow that advice.

If they prescribe another preparation, follow their exact directions.

Dont add potassium permanganate because somebody from your family, WhatsApp group or social media says it worked for them.

A mother recovering in Delhi may need different care from someone in Mumbai, Jaipur, Chennai or Bengaluru because the condition of the wound and recovery can be different.

Plain Warm Water Is Very Different

This distinction matters.

A normal sitz bath can be very simple:

Clean tub → comfortably warm water → soak as advised → gently pat dry → clean and dry the tub.

Potassium permanganate turns that simple routine into a medically directed treatment.

You dont need potassium permanganate just to make your sitz bath a “proper” sitz bath.

In fact, India’s medical-device classification describes a sitz bath itself as a water-filled device used for external hydrotherapy of the perianal and perineal areas.

The water and bathing process are already the basic idea.

The Heal Company – Sitz Bath Tub Manual

At The Heal Company, we designed our product around making warm-water sitz bathing easier at home.

The Heal Company – Sitz Bath Tub Manual has a wide comfortable seat and deep bowl. It is designed to fit most standard Western toilet seats.

Our Manual model uses a hand squeeze pump connected through one pipe to the tub. This lets you create gentle water movement manually without needing electricity.

The Heal Company – Sitz Bath Tub Manual: ₹499

For people who want powered water circulation, we also make:

The Heal Company Automatic Sitz Bath Tub: ₹999

The Automatic version has a motor box with two connected pipes and two separate nozzles. One nozzle draws water from the tub and the second provides the upward water flow. The system operates using the supplied 12V DC adaptor.

The motor box and adaptor should always remain dry.

People contact us from Prashant Vihar and Rohini in Delhi, Andheri in Mumbai, Indiranagar in Bengaluru, Gomti Nagar in Lucknow, Banjara Hills in Hyderabad and Salt Lake in Kolkata.

Many questions are about what should be added to the water.

My view is simple:

Dont add a medical chemical unless your doctor has told you why you need it.

A Recent Safety Update Worth Knowing

There is a genuine safety story around potassium permanganate rather than something we need to invent for this article.

Healthcare authorities issued a major patient-safety alert in 2022 after reports of potassium permanganate being accidentally swallowed. Current specialist pharmacy guidance, fully reviewed in April 2025, still stresses safer prescribing, labelling, storage, preparation and patient education.

That tells us something useful.

Potassium permanganate can have a medical role, but it deserves respect.

It isnt another casual bath ingredient.

When Should You Talk to a Doctor?

Talk with a healthcare professional if you have ongoing bleeding, severe or increasing pain, fever, unusual discharge, increasing swelling, signs of infection or a wound that isnt improving.

And if potassium permanganate has been prescribed, ask your doctor or pharmacist exactly which product to use and how it should be prepared and applied.

Dont guess.

A Note From Abhinav & Deepika

Deepika and I started The Heal Company to develop devices that can support comfort and healing at home.

Our tagline is:

Heal Better. Feel Better.

But when we talk about healing without medicines, we dont mean avoiding medical care.

A sitz bath is supportive care. Potassium permanganate is different because it is an active chemical treatment with real safety risks when handled incorrectly.

For an everyday sitz bath, simple warm water may be all that has been recommended.

If your doctor wants something more, follow them — not a random internet recipe.
