Heal Better. Feel Better
A question we hear around sitz bath use is:
“Can I add apple cider vinegar to my sitz bath?”
It sounds simple. Apple cider vinegar is sitting in many Indian kitchens, and online you can find plenty of claims saying it helps with itching, infection, piles and intimate hygiene.
But something being natural does not automatically mean it belongs on sore or sensitive skin.
I am Abhinav Jain, founder of The Heal Company along with Deepika Jain. While working around sitz bath products, I have learned one thing: people sometimes focus too much on what they can add to the water.
My own approach is much simpler.
For most sitz baths, plain warm water is a better starting point.
Medical sitz-bath guidance says warm water alone works and warns that salts, oils and other substances can cause irritation unless a healthcare professional has specifically recommended them.
So, what about apple cider vinegar? Lets look at the pros, cons and what the evidence actually tells us.
What Is an Apple Cider Vinegar Sitz Bath?
A normal sitz bath is a shallow warm-water bath for your perineal area — the area around your bottom and external genital region.
An apple cider vinegar sitz bath means adding apple cider vinegar, often called ACV, into that water.
You will find many websites and social posts giving their own vinegar-to-water recipes.
I am not giving you one here.
Why?
Because I could not find good clinical evidence establishing a standard apple cider vinegar sitz-bath amount that is proven safe and useful for common conditions like hemorrhoids, anal fissures or postpartum discomfort.
Giving an exact recipe would make it sound medically established when it isnt.
The Possible “Pros” People Talk About
Apple cider vinegar contains acetic acid. This acidic nature is one reason people connect vinegar with cleaning and antimicrobial effects.
But this is where internet advice can get confusing.
Something showing antimicrobial properties in one setting does not mean soaking irritated human skin in it will cure an infection.
And a substance being useful for cleaning something doesnt automatically make it suitable for delicate skin.
So when someone tells me, “Apple cider vinegar kills germs, so it must make a sitz bath better,” I would not make that jump.
For normal sitz-bath care, warm water itself can provide the main soothing effect. Warm-water soaking may ease pain, burning or itching and relax the anal muscles.
That benefit comes from the sitz bath. You dont need to credit the vinegar for it.

Con: Apple Cider Vinegar Can Irritate Skin
This is the part I care about much more.
Apple cider vinegar is acidic.
The skin around the anus, vulva or perineum may already be sore because of piles, fissures, childbirth, surgery, itching or another condition.
Putting an acidic product onto already irritated skin may make things feel worse.
There are published medical case reports of chemical burns following topical apple cider vinegar use.
Those reports don’t mean every diluted use causes a burn. They do show why I dont like treating ACV as a harmless home ingredient just because it came from a kitchen.
Sensitive skin deserves a little more respect.
Con: It May Sting Fissures or Broken Skin
Imagine having a tiny cut on your finger and putting an acidic liquid on it.
Now think about an anal fissure.
An anal fissure is a tear in the lining of the anus. Warm-water sitz baths are commonly recommended as one way to soothe symptoms and relax the muscles around this area.
Adding an acidic ingredient is a different matter.
If you have a fissure, wound, stitches or damaged skin, I would not experiment with vinegar unless your treating healthcare professional has specifically told you to use it.
Plain warm water keeps things much simpler.
Does Apple Cider Vinegar Help Hemorrhoids?
A warm sitz bath may help with discomfort related to hemorrhoids.
That does not mean apple cider vinegar is responsible for the relief.
This distinction matters.
If you sit in warm water and feel better afterward, the warm-water sitz bath itself may be providing the soothing effect.
Medical sitz-bath guidance says you do not need Epsom salt and that warm water alone works; other substances should generally only be added when directed by a healthcare provider.
So I would not advertise apple cider vinegar as a hemorrhoid treatment.
What About Feminine or Intimate Hygiene?
Here I would be even more careful.
Please dont assume that strong, acidic or scented products make intimate areas “cleaner.”
Postpartum guidance, for example, recommends warm sitz baths for perineal discomfort and specifically says not to add bubble bath or other products.
If you have itching, unusual discharge, burning, pain or another symptom, the cause matters.
It could be irritation or an infection, among other possibilities. Using the wrong self-treatment may make irritation last longer.
A sitz bath can soothe an area. It cannot diagnose what is causing the problem.
Can Apple Cider Vinegar Cure an Infection?
I would not make this claim.
An infection needs the correct diagnosis.
Depending on the type and location, it may need medical treatment.
This is especially where internet home remedies can cause confusion. Someone feels burning or itching, assumes it is one condition, and starts adding different things to bath water.
But burning and itching can have many causes.
Dont use apple cider vinegar as a replacement for prescribed medicine or medical care.
That applies even if someone online says it worked for them.
What I Prefer: Plain Warm Water
If somebody asked me what to put into The Heal Company – Sitz Bath Tub Manual, my simple starting answer would be:
Fresh, comfortably warm water.
That is boring advice.
But boring can sometimes be good.
Warm-water sitz bathing is already used for soothing perineal discomfort. Medical guidance describes warm shallow water as the basic sitz-bath method and says additional substances aren’t normally needed unless specifically advised.
Clean the tub before use.
Use fresh warm water.
Sit comfortably.
Afterward, gently pat the area dry instead of rubbing.
Then empty and clean the tub.
That routine makes more sense to me than turning every sitz bath into a kitchen experiment.
Apple Cider Vinegar Sitz Bath After Childbirth
I would be particularly careful here.
After childbirth, there may be swelling, tenderness, stitches or healing tissue.
Warm sitz baths can be used for postpartum perineal discomfort, but medical guidance recommends asking your healthcare provider before starting them after having a baby.
I would not independently add apple cider vinegar to postpartum bath water.
If your obstetrician or another treating clinician has given you a specific bath routine, follow that advice instead.
Apple Cider Vinegar Sitz Bath After Surgery
Same answer.
Your surgeon’s instructions come first.
After rectal, anal or perineal surgery, there may be an open wound or healing tissue. What you put in the water should not come from a random social-media recipe.
Use what your surgical team has advised.
If they say plain warm water, use plain warm water.
The Heal Company – Sitz Bath Tub Manual
At The Heal Company, we designed our sitz bath as a simple home-use device.

It fits on a compatible Western toilet and uses a manual hand pump connected through a pipe to create water movement. The Manual version does not require an electric motor.
We also offer:
The Heal Company Sitz Bath Tub Automatic – ₹999
The Automatic version uses a powered water-flow system.
Whether someone is using a sitz bath in Rohini in Delhi, Jaipur, Lucknow, Ahmedabad, Pune, Bengaluru, Hyderabad, Chennai or Kolkata, my advice about additives is the same.
Dont add something just because it is popular online.
Ask what problem you are actually trying to solve.
My Pros and Cons View
The biggest claimed “pro” of apple cider vinegar is its acidic nature and the antimicrobial claims people associate with vinegar.
But I dont see enough good clinical evidence to recommend an apple cider vinegar sitz bath as a treatment for piles, fissures, postpartum healing or intimate infections.
The cons are easier to establish.
Apple cider vinegar can irritate skin, acidic products may sting damaged tissue, and topical apple cider vinegar has caused chemical burns in published case reports.
For me, that makes plain warm water the simpler choice unless a healthcare professional gives you different instructions.

When Should You Speak to a Doctor?
Dont keep experimenting with bath ingredients when your symptoms are getting worse.
Seek medical advice for worsening pain, increasing redness or swelling, unusual discharge, fever, significant bleeding, or symptoms that aren’t improving.
Medical guidance also recommends contacting a healthcare provider if perineal pain doesn’t begin improving after a few sitz baths.
The goal should be finding the cause, not covering it with another home remedy.
A Note From Abhinav & Deepika
When Deepika and I work on products at The Heal Company, our idea is to make supportive-care devices simple to use at home.
Our tagline is:
Heal Better. Feel Better.
But “healing without medicines” should never mean avoiding medicine when medicine is actually needed.
A sitz bath can support comfort, hygiene and symptom relief for some conditions. It is not a replacement for medical treatment.
And sometimes the simplest sitz bath is the one I like best:
A clean tub. Fresh warm water. Nothing unnecessary added.
